Fibrotic reaction to hyaluronic acid injectable implants in the face

Background

Hyaluronic acids (HA) can have very different effects, which depend not only on the physician and the patient’s body, but also on their molecular weight.

  • Short-chain HA exhibits immunological activity.
  • Long-chain HA affects fibroblasts and may stimulate them to produce collagen.

Although this is usually considered positive, it can be unfavorable in certain anatomical areas.

Patients and Method

We treated 23 patients who developed fibrosis next to the nasolabial folds, which:

  • Accentuated the folds
  • Became very noticeable when smiling

Injection of hyaluronidase did not reduce this mass.

Results

Intralesional injection of triamcinolone acetonide led to rapid improvement.

Discussion

Fibrotic tissue reaction, which does not respond to hyaluronidase, can be a complication of hyaluronic acid injection.

This condition can be effectively treated with intralesional corticosteroid injection.
