Reconstruction with flaps in the knee region: Review of current perspectives

A review of the international literature on reconstruction with flaps in the knee region revealed significant disagreements regarding the optimal management of defects in this area.

  • Muscle flaps remain the preferred option, especially in cases of infected wounds.
  • Perforator flaps have recently been introduced as a coverage option for the knee, offering significant advantages such as:
    • Low donor site morbidity
    • Long pedicle and wide arc of rotation

When a free flap is chosen, the selection of recipient vessels is a decisive factor for the success of the reconstruction.

Based on published experience, the authors propose a reconstruction algorithm based on the following criteria:

  • Presence of a three-dimensional defect
  • Size and location of the wound
  • Presence or absence of infection
